This 6-month membership includes:
One 90-minute intake visit
Three 60-minute lab review follow-up visits (in-person or telehealth)
Secure text and email support during business hours
Pricing:
$320 initial comprehensive visit
$199 per month starting the following month (6-month commitment)

Our services are concierge membership and not billed to insurance. You can however use insurance to pay for recommended labs, imaging, and prescriptions if needed.

Email and patient portal messaging support Monday through Friday, 9:30 AM – 2:00 PM
Responses to messages with a 24-48 hr response time during business hours
This membership does NOT include:
Primary care services
Urgent care services
Emergency medical services
After-hours or on-call coverage

This membership is not fee-for-service. Monthly fees are charged regardless of appointment attendance or scheduling. If you are unable to make, keep, or schedule your appointment during the appropriate month, you will still be charged on your regular billing date.

After completion of the initial 6 months, you may choose to continue or discontinue membership.
