This Membership Agreement outlines the terms and conditions for participation in the Fireweed Functional Medicine Membership Program. Please read carefully and sign below to indicate your understanding and agreement.
1. Membership Fees and Billing
The membership fee is $320 for the first month.
Ongoing membership is $199 per month, beginning the second month.
This membership requires a 6-month commitment.
After completion of the initial 6 months, you may choose to continue or discontinue membership.
By enrolling, you authorize Fireweed Functional Medicine to charge the monthly membership fee to your provided payment method on file on a recurring basis.
This membership is not fee-for-service. For reference, our fee-for-service visits are $515 per follow-up visit.
Monthly fees are charged regardless of appointment attendance or scheduling. If you are unable to make, keep, or schedule your appointment during the appropriate time, you will still be charged on your regular billing date.
2. Services Included in Membership
Your membership includes:
One 90‑minute Initial Intake
Comprehensive history
Functional timeline
Systems review
Initial plan and labs ordered
Three additional 60‑minute Lab Review Follow-Up Visits
Email and patient portal messaging support Monday through Friday, 9:30 AM – 2:00 PM
Responses to messages within 24-48 business hours
Communication outside of these hours will be addressed during the next business window.
3. Services Not Included
This membership does NOT include:
Primary care services
Urgent care services
Emergency medical services
After-hours or on-call coverage
Fireweed Functional Medicine is not a replacement for a primary care provider. You are responsible for maintaining a relationship with a licensed primary care provider and for seeking urgent or emergency care when appropriate. In the event of a medical emergency, call 911 or go to the nearest emergency department.
4. Nature of Membership
This is a membership-based functional medicine program. Payment covers access to care and services as outlined above, not individual visits or discrete medical services. Unused visits do not roll over.
5. Cancellation Policy
You are financially responsible for the full 6-month commitment. After the initial 6 months, you may cancel your membership with written notice prior to your next billing cycle.
Fireweed Functional Medicine reserves the right to terminate membership with appropriate notice.
6. Acknowledgment and Consent
By signing below, you acknowledge that:
You understand the membership fees and billing structure.
You understand that this is not fee-for-service and that charges occur regardless of appointment use.
You understand that this membership does not provide primary, urgent, or emergency care.
You agree to the 6-month commitment and recurring billing terms.
